Scheduling Appointments

Please call early in the day for same day sick appointments. Routine well child appointments should be scheduled as soon as you get a reminder notification.Cancelling AppointmentsScheduled appointments should be cancelled at least 24 hours in advance to allow the office to offer this appointment to another patient

Missed Appointments

Missed appointment including appointments cancelled less than 24 hours in advance put a strain on the office. Repeated missed appointments will be grounds for ending your relationship with this office.

Minor children

Children under 18 years are expected to be accompanied by a parent or guardian. If a parent/guardian is unable to bring the child to his/her appointment, a Caretaker Authorization form should be completed. A parent/guardian also needs to consent to care including vaccinations for all minor children.

School and Camp Forms

Please request medical forms at least 24 hours in advance, more advanced notice is recommended in case the office is closed for vacation. We recommend copying and saving your completed forms in case they are needed for other schools/daycare, sports or camps.

Prescription Refills

Please call your pharmacy to refill prescriptions; they will contact our office if refill authorization is required. For refills which require a written prescription such as controlled medications for ADHD, please provide at least 24 hours’ notice, more advanced notice is recommended in case the office is closed for vacation.

Phone CallsIf you have a non-urgent medical question, please call the office during business hours. The physician will return your call by the end of the day.
